Daily Health Assessment

NYX’s first priority is for the safety and health of our employees, customers, vendors, visitors, their families, as well as our surrounding communities. We greatly appreciate your daily participation in our self-declaration screening process as one of the precautionary measures we can take to prevent and limit the risk of exposure and illness. The information that you provide will be used to determine your access rights to NYX facilities.
We ask for your honest answers to the following questions relative to symptoms including, but not limited to:
• Temperature of 100.4F/38C or higher
• Cough (frequent)
• Shortness of breath or difficulty breathing
• Fatigue or tiredness that can’t be explained
Question 1: Have you had physical exposure to a person suffering from SARS-CoV-2 symptoms as noted above?
Question 2: Have you or anyone in your household experienced any of the following symptoms in the last 14 days?
Question 3: Have you or anyone in your household traveled internationally or been on a cruise in the last 14 days?
Question 4: Have you or a household member been in an enclosed place with 10+ people in close proximity without wearing a mask?
Question 5: Are you, or anyone in your household, actively waiting upon the results from a COVID test?

Each day NYX will ask you to respond in one of the two following ways:
1. No – to all questions
2. Yes – to one or more questions
